In v11 you have the ability to mark the next stage of a problem as transitioning from it's previous stage. I have attached an example using Chronic Kidney Disease Stage 1 that transitions to Chronic Kidney Disease Stage 2.

1. Add Chronic Kidney Disease Stage 1 to your Active Problem list (commit) 2. Add Chronic Kidney Disease Stage 2 to your Active Problem list 3. Edit Chronic Kidney Disease Stage 1 by adding “Transitioned to:” and by changing status to resolved.
4. Result should be that Stage 2 is in the Active List and marked as “transitioned from Stage 1” and Stage 1 should move to PMH List.
