Q: Why are document types missing from the document type dictionary even though end users can see the note templates attached to those document types?

This can happen when converting notes from V10 to V11 when the V10 document type has the exact same name as the V11 document type. Although the two document types are different manifestations, when using CMT to move the newly created V11 notes and their associated document types, it appears that system confusion is created because of the duplicate naming convention. This occurs in spite of the fact that the codes and mnemonics are different.

To avoid this problem, ensure that newly created documents types (with a Noteform manifestation) do not have the exact same name as existing document types of other manifestations.
