Plan section missing from note output V11.1.6

This issue occurs when the clinical staff person starts the note from an arrived patient pulled from the provider schedule. The note is then closed and the user logs out. The doctor then logs in, clicks on the note icon from the schedule, goes to the plan section of the note, clicks "new" and enters orders from that screen.

The workaround would be to use the clinical toolbar to place orders instead.
