Introduction

The Health Management Plan or HMP component of the Clinical Desktop is a tool used to prescribe and monitor events related to a patient’s health maintenance and disease management. It enables users to view and modify health maintenance items for the selected patient. For instance, if a patient has Asthma, a doctor might utilize the HMP module to review the patient’s albuterol dose, confirm the patient has had an influenza vaccine, monitor the patient’s symptoms in a flowsheet, and setup a yearly reminder for a flu shot.
The Health Management Plan webcast defined the Health Management Plan or HMP component of the clinical desktop and reviewed the purpose and different components. Throughout the presentation we will use a test system was used to demonstrate different functionality within the HMP component.

6 Main Views of HMP Component

There are six main views in the HMP Component

  • Health Management - The Health Management view is a valuable tool that allows the user to quickly review and manage problems, current medications, reminders, and recent results for the selected patient
  • Reminders/Alerts - The Reminders/Alerts view displays all active Order reminders placed on the patient’s chart.
  • Flowsheets - The Flowsheets view is used to easily view data related to a specific visit type or disease management.
  • Normative Growth Chart - The Normative Growth view displays growth data supplied by the Center for Disease Control for individuals 20 years and younger. This allows the provider to compare the patient’s percentages to the CDC standards.
  • Immunization - The Immunization view is a comprehensive view of all historical vaccines for a patient.
  • Vital Signs - The Vitals Signs view is a flowsheet of the patient’s vitals data.

Q&A

QUESTION: Can you print the HMP? Is there a way to print the Health Management Plan from the ACI? I see where you can print flow sheets, but the Print function is greyed out for the HMP component so it is not a choice. Printing would be nice. It would allow providers to have a list of outstanding items needing be addressed at an appointment. Yes the physician should be looking at the screen but there are many who do not take the computer into the room with them.

  • ANSWER: Currently, there is not functionality to print the HMP component from the clinical desktop. I’ve seen enhancement requests on ClientConnect for this, so hopefully it will be a part of 11.3. You can; however, print by using local printing: i.e.: print preview, “only print selected frame” and 100% in landscape and it prints okay. This could be a workaround until able to print from EHR.

QUESTION: I upgraded from v10 to v11 and none of my “Last Done” dates are displaying for my order reminders. How can I re-enter this data?

  • ANSWER: If results were scanned into the system for these orders or the orders came over the interface, you can reference chartviewer to get the Last Done date. If you want to update the Order Reminder, you will need to right click in the To Do field and choose “Completed On”. A window will display that will allow you to choose a date in the past in which the order was created.

QUESTION: How can we satisfy the completion of non-interfaced items such as a foot exam (referral), opthalmology exam, etc? I would presume that this could be done when the consult letter/result is being scanned in. I'm new to Allscripts THX!

QUESTION: We have not been using HMP since outr upgrade from V10. What is your best suggestion for making the HMP a user friendly tool again with out the clutter of information that is presently there? Am I understanding this that we will have to update over 50000 records by hand???? QUESTION: Are there any scenarios in which deferring a reminder would be useful (from the to do column)? I have not been able to think of a good example to train our physicians.
