Description

Patient Health Questionnaire (both version 2 and 9). This is used as a tool determine the severity of depression.

Functionality

The questionnaire will initially be in the PHQ-2 version. Should the user select an answer greater than zero, the questionnaire will switch to the PHQ-9 version.

Calculator Search/Filter Keywords
