Description

During this webcast, we addressed the Chronic Care Management and Transitional Care Management programs and the criteria needed to bill for them. We focused mostly on CCM Care Plans and Follow Up and the documentation required to substantiate using the CPT Code 99490.

Q&A

Question: Is there regulation on who can intiate the care plans with a patient? Does it have to be a RN?

Answer: Ultimately the provider has to sign/approve the care plan per guidelines. However, clinical staff/RN may add content but the care plan has ot be managed by a MD, DO, NP or PA.


Question: If you open the wrong template or duplicate template is there a good way to cancel or void that incorrect one?

Answer: If unsigned, the “Cancel” radio button can be selected when the Sign document button is activated. In the event that a signed document needs to be cancelled the cancel button will not be available for a signed document, however, your MPM clinical analyst does have the ability on the back end to go thru the RAD>Transcriptionist Desktop where they can unsign the document and then change the status to Cancelled.
