Med Admin creates a Med Admin Encounter instead of linking to Appointment Encounter

This is the result of a recent code change (V11.1.7) and is WAD. In order to get the Med Admin charge to link to the appointment encounter, both the Billing Provider and the Billing Location must match the appointment encounter.
