To assess a problem in V11.4, click the paper icon to the left of the problem name from the clinical desktop or from the Add Clinical Item (ACI). Problems can also be assessed from the V11 note by adding in a note form with the top diagnosis list to the assessment section of the note and then clicking the check box next to the problem to be assessed.
